ANNUAL EVALUATION

Every 12 Months, The Celebrity Limo Group conducts Annual Evaluations of all drivers of the previous year, as in accordance with FMCSAR 391.25b. At this time, we review Certifications of Road Tests, Driving Records, Customer Evaluation Reports and USDOT Documents that are required by law to operate a comercial vehcile in accordance to the FMCSA. Each driver is required to report any moving violations that he or she has received. Management must then decide if he or she is still qualified to drive a Commercial Vehicle in accordance with the FMCSA.

Each driver, during his employment, will be evaluated annually. This Driver Annual Evaluation Form will be added to his/her Driver Qualification File in accordance with the FMCSAR 391.25b. This form is only to be completed by the Employee.
