Drug & Alcohol Clearinghouse Consent Form - PDF Below

This form provides consent for Celebrity Coaches to conduct a limited query of the FMCSA Commercial Driver’s License Drug & Alcohol Clearinghouse to determine whether drug or alcohol violation information about you exists in the Clearinghouse. This consent will include an initial query as well as an annual query for the duration of your employment with the company.

​In accordance with FMCSAR 382.61, Celebrity Coaches has a Controlled Substance & Alcohol Use and Testing Driver Awareness Program.  All CDL Drivers must read our Driver Awareness Information Packet (PDF Below) and sign the Driver Awareness Acknowledgement Form (PDF Below) to comply with all company policies and procedures as they relate to the FMCSAR regulations.
